Ask for prescription refills at your healthcare appointment. For other prescription refills, call Lifecare at 330-454-2000 ext. 600. Prescription refill requests are processed during Lifecare’s business hours.

Prescription Refills

When calling in for your prescription refill, please provide the following information found on your medicine bottle:

  • Your full name (please spell your last name)
  • Date of birth
  • Phone number where we can reach you
  • Name of the medication (spell the medication)
  • Strength or dosage
  • Directions for your medication
  • Pharmacy name
  • Pharmacy telephone number

Please allow for two business days for refills to be processed. After two days, check with your pharmacy to see if your prescription is ready. If your prescription is not at the pharmacy after two days, call Lifecare to schedule an appointment.

We do not accept phone calls or faxes from your pharmacy. All narcotics and antibiotic requests require an appointment.
