Reduced Rate Program

Lifecare offers reduced rates for qualifying patients based on household size and income. If you are interested in applying for our Reduced Rate Program, download and print the Reduced Rate Application. Patients qualifying for reduced rates must reapply every year, before April 1, or at the time of the year’s first visit.
