Our Community Health Worker can help reduce health disparities by providing suggestions on how to improve a woman’s quality of care, and empower her to strive for better health.
The Community Health Worker will meet with the pregnant woman to complete a comprehensive assessment and screening to determine if there are any social or physical health risks she may be experiencing.
She provides support and personalized visits to pregnant women during and after pregnancy, from birth through the first year of life.
