Our team of health care professionals includes family physicians, certified nurse practitioners, dentists, dental hygienists, optometrists, behavioral health counselors, and a psychiatric nurse practitioner. 2019 MARKETPLACE OPEN ENROLLMENT STARTS IN NOVEMBER
2019 Marketplace Plans
- Lifecare’s Certified Application Counselors are qualified and available to discuss options for the Health Insurance Marketplace Plans (Healthcare.gov) Monday through Friday 8:00 am to 3:00 pm.
- The 2019 Open Enrollment Period runs from November 1, 2018 to December 15, 2018.
- Plans sold during Open Enrollment start January 1, 2019.
Ohio Medicaid
Those who are interested in completing a Medicaid application are encouraged to check their eligibility first with a Certified Application Counselor (CAC) at Lifecare.
Lifecare Family Health & Dental Center has been designated as a Qualified Provider to make Medicaid eligibility determination for an applicant. CACs at Lifecare have the ability to issue Presumptive Eligibility (PE) for Ohio Medicaid. (Presumptive Eligibility is an expedited process of enrolling eligible applicants in the Medicaid program. PE gives uninsured people immediate, temporary Medicaid if they appear to be eligible based on income.)
